For adults with disabilities, there are many different ways to receive services outside of our centers. Through our community-based services, individuals are able to learn how to navigate real-world situations while receiving care aware from our center.
Our Programs
Respite
For parents or caregivers who need a moment to take care of personal responsibilities, we will match the individual with a provider. The provider may choose to spend a few hours or a few nights with the individual.
Community Living Supports/Personal Assistance
For adults with disabilities, Community Living Supports (CLS) and Personal Assistance (PA) provide one-on-one assistance in areas of daily living and offer opportunities for therapeutic recreation. This service can be provided in an individual's home or in the community. CLS/PA enables a person to accomplish tasks they would be able to do on their own without a disability, including personal care and money management. The providers give hands-on assistance, guiding, and training during activities. It is the goal of the provider to ensure individual goals are being met. Our CLS/PA providers will assist with medical care and transportation for the individuals to appointments, activities, or community services.
Michelle P. Waiver is required to participate in CLS. Supports for Community Living (SCL) Waiver is required to participate in PA.
Foster Home Providers
Foster Home Providers (FHP) give residential care to eligible adults who need support for daily living.
