Case Management

Easterseals provides case management services through qualifying waivers.

Overview

Easterseals provides traditional Case Management services for Michelle P (MPW), Supports for Community Living (SCL), and Home and Community Based (HCB2) waivers. 

The main goal of Case Management is to work closely with and advocate for the participant to ensure they are satisfied with their services and their needs are adequately met. Case Managers can also assist in application for waivers and in locating and accessing community resources. 

The health, safety, satisfaction of services, and welfare of each person we serve is our ultimate priority. We ensure needs are met through monthly visitations, whether in person or via phone call.

Easterseals West Kentucky wants to make sure that each individual has access to the services that are appropriate for their needs. We want each individual to be able to utilize services that develop independent living skills and confidence. We help to determine and meet their life goals while ensuring they are living a fulfilling life.

    Adult Day Health Care

    Offering compassionate, medically supported care that enhances health, independence, and connection for adults with diverse needs.

    Adult Day Training

    Adult Day Training provide an opportunity for adults with disabilities to create new experiences, become more independent, and build lasting relationships.

    Autism Services & Therapy

    The Lori Center for Autism provides one-on-one Applied Behavior Analysis (ABA) therapy for children ages 2-12 with a diagnosis of Autism Spectrum Disorder.

    Behavior Support Services

    We provide positive behavior supports to help promote independence, identify target behaviors, and utilize strategies for controlling behavior responses.

    Early Childhood Care

    Our Early Childhood Education Center supports all abilities, nurturing each child's growth and potential at their own pace.

    Employment Connections

    We provide Supported Employment and Employment & Retention for individuals with disabilities who are eager to find meaningful employment.

    Waiver Assistance

    A waiver is often required to receive many of our other services. We provide experts who assist with waiver applications and who provide support during long wait-times.
